Ballantyne and Fort Mill ADVANCE- Haiti, June 2018
Welcome to the ADVANCE June 2018 Forest Hill Haiti Trip page!
Our group of students from Forest Hill Church Ballantyne and Fort Mill will be traveling to Titanyen, a village just north of Port-au-Prince with 13,000 residents, many of which resettled in the area after the earthquake. Our campus global partner, Mission of Hope, meets the physical and spiritual needs of the local communities by providing education about better hygiene, meeting the immediate physical needs of the village, and helping the church to play an integral part of empowering the community. Please join this trip or donate to help each of us go deeper in Christ as we follow His command to be His Church.
Trip Focus:
This is a FHC ADVANCE High School Ministry trip and is only open to students, rising 9th grade and above. The team will serve alongside Mission of Hope in supporting the community and sharing the message of Christ's love.

FHC & MOH Liability Waiver Required

The FHC & MOH liability waivers must be completed for every trip, regardless of past participation.
Background Check Required
Due 3/23/2018
A new background check is required for all trip participants who will be 18 years or older by June 30, 2018.
Parental Consent Form Required
Due 3/23/2018
This form will need to be Notarized by BOTH parents.
Requirements
Prerequisites
- Youth must at least be a rising 9th grader
- Must have a valid passport that does not expire before December 30, 2018
- Must serve 2 times locally before trip
Preparation
- Apply for the trip and pay the $110 non-refundable deposit
- Fill out the forms under the Forms tab
- Create personalized trip page
- Check the CDC website for updated information on recommended immunizations http://wwwnc.cdc.gov/travel/
- Commit to attending 6-8 team meetings, including the debrief meeting after the trip returns
- If you sponsor a child through MOH and would like to visit them on your trip, please login into your MY MOH account to sign up for a visit or send an email to [email protected] at least 8-weeks prior to your trip date.
- Make sure 100% of all trip fees are submitted 10 days prior to departure
