The FHC liability waiver must be completed for every trip, regardless of past participation.
Background Check Required

Before you complete a new form - If you have traveled on a recent trip, or currently volunteer in the FHC Kids ministry, please let your trip leader know so that we may verify if we have a valid background already on file.
SAM Waiver Form Required
Due 8/1/2016
The SAM liability waiver must be completed for every trip, regardless of past participation. This form will need to be notarized.
3 Colored Copies of Valid Passport Required
Due 8/1/2016
Before you submit a copy of your passport - If you have traveled on a recent trip, please let your trip leader know so that we may verify if we have a copy of a valid passport already on file.
Requirements
Prerequisites
- Must have a valid passport that does not expire before April 11, 2017
Responsibilities
- Each particpant will commit to attending 6-8 team meetings
- 100% of trip fees are due 10 days prior to departure, or participant is ineligible to travel
Restrictions
- Trip Participants must be 18 years or older
Preparation
- Please check the CDC website for updated information about immunizations http://wwwnc.cdc.gov/travel
- Commit to reading "Helping without hurting in short-term missions" by Brian Fikkert
- Commit to watching the 6-part video series on "Helping without hurting"
- Commit to watching the documentary "Poverty Inc"
